Intake
Counselling, consultation and coaching services usually involve an intake stage, where the professional and customer make acquaintance with each other and establish the basics and the goals of their work together during the forthcoming sessions.
In order to reduce your cost and expedite the preparation for therapy, we ask you to go through a list of possible questions and issues that will help you better describe your situation and set your therapy goals. We will not make you answer any question or address any issue included in our general intake form, we only give it to you as a guideline. You will have to decide how much information you disclose, how fast you will go, and how deep you will dig into your life during therapy.
For couple, family or group counselling, each participant - who is physically and intellectually capable of doing so - will need to complete a separate intake form, without any influence or monitoring by another participant.
